You can check your ๐—˜๐—ข๐—ฃ๐—™, which is your Electronic Official Personnel Folder, for ALL the beneficiary forms youโ€™ve ever submitted over the years.

Anyone can be a Beneficiary. They could be a relative, a friend, a church, a library or even to a trust or any combination of those things. By submitting a new beneficiary form, youโ€™re casting all of your previous designations on the beneficiary forms. Beneficiary Forms are only valid once theyโ€™ve been received by your HR office, signed and made part of your EOPF.

Youโ€™re allowed to change your beneficiary forms as much as you likeโ€”it doesnโ€™t matter how many times you do itโ€ฆanytime you want to!

Use the:

  • ๐—ฆ๐—™๐Ÿฎ๐Ÿด๐Ÿฎ๐Ÿฏ for life insurance (FEGLI)

  • ๐—ฉ๐—ฆ๐Ÿฏ๐Ÿญ๐Ÿฌ๐Ÿฎ for the Civil Service or the FERS Retirement Funds

  • ๐—ฆ๐—™๐Ÿญ๐Ÿญ๐Ÿฑ๐Ÿฎ for Unpaid Compensation, like a paycheck or payment for your vacation days

  • ๐—ง๐—ฆ๐—ฃ๐Ÿฏ for the Thrift Savings Plan funds that must be completed through TSP.gov

and should never be submitted to your federal agency
